Minor Release and
Waiver Details

This Release and Waiver of Liability (the "release") executed by [minor "Visitor/Volunteer" printed name] and [Parent of minor "Visitor/Volunteer" printed name] releases Lyric Farm/The Refuge at Lyric Farm ("Entities"), and each of its directors, officers, employees, agents, and volunteers. The minor Visitor/Volunteer desires to visit and/or provide volunteer services for Entities and engage in activities related to being a visitor and/or volunteer (the "Activities").

Minor Visitor/Volunteer and parent understand that the scope of Visitor/Volunteer's relationship with Entities is limited to a visitor/volunteer position and that no compensation is expected in return for visits and/or services provided by Visitor/Volunteer, that Entities will not provide any benefits traditionally associated with employment to Visitor/Volunteer, and that Visitor/Volunteer is responsible for his/her own insurance coverage in the event of personal injury or illness as a result of Visitor/Volunteer's visit and/or services to Entities.

WARNING

Under Massachusetts law, an equine professional is not liable for an injury to, or the death of, a participant in equine activities resulting from the inherent risks of equine activities, pursuant to section 2D of chapter 128 of the General Laws.

1. Waiver and Release

We, the minor Visitor/Volunteer and parent of the minor Visitor/Volunteer, release and forever discharge and hold harmless Entities and its successors and assigns from any and all liability, claims and demands of whatever kind or nature, either in law or in equity, which arise or hereafter arise from the visit and/or services my minor may provide to Entities. We understand and acknowledge that this Release discharges Entities from any liability or claim that we may have against Entities with respect to bodily injury, personal injury, illness, death or property damage that may result from the visits and/or services minor may provide to Entities or occurring while minor is visiting and/or providing volunteer services.

2. Insurance

Further We understand that Entities do not assume any responsibility for or obligation to provide us with financial or other assistance, including but not limited to medical, health or disability benefits or insurance of any nature in the event of minor's injury, illness, death or damage to minor's property. We expressly waive any such claims for compensation or liability on the part of Entities beyond what may be offered freely by Entities in the event of such injury or medical expenses incurred by minor.

3. Medical Treatment

We hereby Release and forever discharge Entities from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during minor's visit and/or tenure as a volunteer with Entities.

4. Photographic Release

We grant and convey to Entities all rights, titles, and interests in any and all photographs, images, video, or audio recordings of minor or minor's likeness or voice made by Entities in connection with minor's visits and/or providing volunteer services to Entities.

5. Other

As a minor visitor and/or volunteer and parent of a minor visitor/volunteer, we expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Massachusetts and that this Release shall be governed by and interpreted in accordance with the laws of the State of Massachusetts. We agree that in the event that any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected.
